Hospital In the home for mater cancer care patients (#366)

New Provision of Care for patients in Mater Hospital South Brisbane.

Hospital in the Home (HITH) program provides eligible patients Cancer treatment in their homes as an alternative to being treated in hospital. Hospital in the Home program is available to all age groups and patient eligibility is determined by the patients authorised medical practitioner and distance from the Hospital.

Mater Cancer Care Centre (MCCC) is a specialised 40 Chair, Day Cancer Unit offering services for Oncology, Haematology, Apheresis and Palliative care for Patients affected by Cancer. 

Aim: 

To provide services for patient's being treated with Folfox6 or Folfiri protocols, the opportunity to have their Day 3 Fluorouracil Infusion disconnected in the patients home.

Outcome: 

Initial project commenced with patients due for weekend disconnection and after a 4 week trial, is now offered to all eligible patients 7 days per week.

Summary:

Mater's HITH program, in partnership with MCCC, now provide a quality health professional team, replicating service in the hospital in the comfort of the patient’s home. This gives patients greater independence, choice and comfort.
